Dr. Dhaval Shah and Dr. Amita Ghia both are USCIS designated Civil Surgeons.
We perform immigration physicals at both of our locations: Bethelview office and Old Atlanta Road office.
Time
We usually provide same day appointments. After the initial appointments, you may have to come back for a chest x-ray if your tuberculosis test is positive. Once we have results blood tests and urine test which typically take 3 days, we will provide you your forms in a sealed envelope for submission to USCIS.
Checklist of Required Documents:
- USCIS Form I-693: Please download it from here. Fill out only part 1. (Do NOT sign it.) If you don’t have access to a printer, we will provide you with the form when you are in the office.
- Proper identification (Photo ID): Passport (preferred) or one of the
following : Driver’s license, Green card, Birth Certificate, State issued ID or Military ID card. - Alien Registration Number: this is optional.
- Please bring with you:
- Medical records (translated into English, if necessary)
- Immunization records (translated into English, if necessary)
